Please note that completion of this referral form is for scheduling services with Arrow Diagnostic Imaging only. Scheduling an imaging appointment with Arrow Diagnostic Imaging does not include a surgical consultation. If a surgical consultation with Phoenix Veterinary Surgical Specialists (PVSS) is also desired, this must be arranged directly through a PVSS front desk associate. The fee for a consultation with a board-certified surgeon at Phoenix Veterinary Surgical Specialists is $150. This appointment may be scheduled on the day of imaging especially if there is a question of which imaging modality is needed. However, in many cases, it is often more beneficial for the patient and client to meet with a surgeon, after imaging has been completed and the radiology reports have been finalized.
As Arrow Diagnostic Imaging is an outpatient imaging facility, we do not provide ongoing treatment or hospitalization. If treatment is required while a patient is in our care, the owner will need to transport their pet either back to your facility or to a local emergency veterinary clinic.
If you have an urgent imaging referral that you feel may also be a potential surgical PVSS referral, we recommend coordinating this through Phoenix Veterinary Surgical Specialists directly. The PVSS team will then coordinate imaging through Arrow Diagnostic Imaging as appropriate. This helps tremendously with owner expectations and communications.
